The problem

Rework is the least measured cost in construction. It is absorbed into labour hours and material wastage, and because it is never isolated, nobody can say what quality failure actually costs the project.

The same is true of safety: incidents are reported, and the leading indicators that would have predicted them — inspections carried out, permits closed, observations raised — are not.
